Complete and submit the form

1. Complete and sign the waiver electronically using a PDF editor, or print it and fill it out by hand.
2. If you complete it by hand, scan the signed form as a PDF or clear image.
3. Attach the completed and signed Activity Participation Form when submitting the VMUNC VIII Registration Form.
