Activity Participation Form
Required for every delegate
All delegates must complete the VMUNC Activity Participation Form. Delegates without a completed form will not be able to participate in VMUNC VIII.

- Complete and sign the waiver electronically using a PDF editor, or print it and fill it out by hand.
- If you complete it by hand, scan the signed form as a PDF or clear image.
- Attach the completed and signed Activity Participation Form when submitting the VMUNC VIII Registration Form.
